Breastfeeding and Work : Lets Make It Work !

Breastfeeding is one of the important determinants of child health, development, nutrition and survival. Now, more women are working outside the home both in formal and informal sector. Difficulties working women face in practicing breastfeeding are also increasing. It becomes critical to build an enabling system that would allow combining breastfeeding and work. In this context, maternity protection policy is vital for all women; a policy that should entitle all women to work with dignity, and to benefit from respectable work conditions that bar out discrimination based on gender and their reproductive role. The Constitution of India, under article 42, states, “The State shall make provision for securing just and humane conditions of work and for maternity relief.” In light of the Constitutional mandate, India has two legislations for working women. One, the Maternity Benefit Act 1961 and second the Employees State Insurance Act, 1948. India has also in place some executive measures based on recommendations of the 6th Pay Commission. Further, the National Food Security Act 2013 recognizes maternity protection a legal right via Section 6 providing for a minimum of Rs. 6000/- to be given as maternity entitlement to all women, which includes women from the unorganized sector. The theme for this year's World Breastfeeding Week is . It revisits the 1993 WBW campaign on the Mother-Friendly Workplace Initiative. It aims to empower and support all women, working in both formal and informal sectors, to optimally combine work with child-rearing, particularly breastfeeding. The theme for 1993 WBW was “Women, work and breastfeeding: Everybody benefits!
OBJECTIVES 
  1. Inform people about the available maternity protection entitlements and raise awareness to strengthen the national legislation and implementation. 
  2. To gain multi-dimensional support from all sectors to enable women everywhere to work and breastfeed. 
  3. Promote action by employers to become baby and mother-friendly and actively facilitate and support their women employees to continue breastfeeding. 
  4. Strengthen supportive practices that enable working women in the informal sector to breastfeed.

World Breastfeeding Week-2015

WBW is the World Breastfeeding Week which is celebrated annually from 1st August to 7thAugust in almost all the countries. The variety of events on breast feeding has been exhibited by many countries, organizations and participants worldwide.
World Breastfeeding Week 2015 would be celebrated from Saturday (1st of August) to the Friday (7th of August).
Why it is celebrating
Breast feeding is organized and promoted worldwide by the WABA (World Alliance for Breastfeeding Action), WHO (World Health Organization) and UNICEF (United Nations International Children’s Emergency Fund) to get the goal of elite breastfeeding by mother for their baby of first six months in order to get the incredible health benefits, to fulfill the all vital nutrients, to encourage mother for the healthy growth and development of their child, to guard them from the lethal health problems and diseases including neonatal jaundice, pneumonia, cholera and many more.
It was first started and celebrated by the World Alliance for Breastfeeding Action in the year 1992. And later it is being celebrated in more than 120 countries by the UNICEF, WHO and their participants such as individuals, associations and governments. World Alliance for Breastfeeding Action was also established in the year 1991 at 14th of Feb to promote the comprehensive breastfeeding culture worldwide by providing the support and achieve the real goal.
The breast feeding has been emphasized to a great extent by the World Health Organization and the American Academy of Pediatrics (AAP) for promoting the healthy living for both the mother and the baby. It is highly recommended by them to all mothers that they should give breastfeeding to their child for the first six months of the life and after that a supplemented breastfeeding can be given for the one, two or more years.
The World Alliance for Breastfeeding Action had assisted and supported the social mobilization for the global promotion of the campaign of breastfeeding. WHO and WABA had decided a day committed to the breastfeeding which has been marked as one of the important international events.
It is very necessary to the mothers to get timely support and healthcare education to their baby including breast feeding. The continuous support and intimation can bring the gradual and permanent changes in the social living and ultimately healthy and disease free social living. To sustain a continuous breast feeding can be traditionally supported by the family, community leaders, trained health workers, lactation consultants, friends and partners.
The strategy of peer counseling can be beneficial and exceedingly productive approach to distribute the message of breast feeding campaign to a larger number of other worldwide in a less time period. The trained person of the community can be a peer counselor which can support the mothers to combat their breastfeeding questions and issues.
Aims of the Breast Feeding Campaign
To make aware the peer group to support mothers in order to establish and carry on the breastfeeding.
To initiate the breastfeeding supporters to be trained to provide support to mothers and babies in more effective ways.
To make aware the people to attend and expand the Peer Counseling programmes by letting them know the effective and efficient benefits of the Peer Counseling.
To call on the governments to get more and worldwide maternity facilities in order to increase the rate and duration of elite breastfeeding.
To discover the contacts of the neighboring community support so that breastfeeding mothers can go them to get help and support after delivery.
There are five circles of support from which a mother should get assistance for breast feeding. A woman is surrounded by these circles of support which are Family and Social Network, Workplace and Employment, Government or Legislation, Healthcare and Response to Crisis or Emergency.

स्तनपान केवल पोषण ही नहीं, जीवन की धारा है


स्‍तनपान शिशु के जन्‍म के पश्‍चात एक स्‍वाभाविक क्रिया है। परन्‍तु पहली बार माँ बनने वाली माताओं को शुरू में स्‍तनपान कराने हेतु सहायता की आवश्‍यकता होती है। स्‍तनपान के बारे में सही ज्ञान के अभाव में जानकारी न होने के कारण बच्‍चों में कुपोषण का रोग एवं संक्रमण से दस्‍त हो जाते हैं।  स्तनपान शिशु के लिए संरक्षण और संवर्धन का काम करता है। मां का दूध (स्तनपान) केवल पोषण ही नहीं, जीवन की धारा है,  ज़िन्दगी के लिए अमृत समान  है.। इससे मां और बच्चे के स्वास्थ्य पर सकारात्मक प्रभाव पड़ता है।
क्यों ज़रूरी है स्तनपान   
  • स्तनपान शिशु के लिए संपूर्ण पूरक और पोषक आहार है।
  • यह हर मौसम में सही तापमान पर बच्चे के शरीर में पहुंचता है।
  • स्तनपान करने वाले शिशु सामान्य शिशुओं की तुलना में कम बीमार पड़ते है। 
कोलस्ट्रम और उसके फायदे
मां के पहले दूध में कोलस्ट्रम पाया जाता है। कोलस्ट्रम पीले रंग का गाढ़ा दूध होता है, जो कि शिशु को रोगों से लड़ने की क्षमता प्रदान करता है। बच्चे के जन्म के दो से चार दिन के बाद यह सामान्य दूध में बदल जाता है। समय से पहले पैदा हुए बच्चों के लिए कोलस्ट्रम बहुत आवश्यक है।
  • कोलस्ट्रम में अधिक मात्रा में प्रोटीन, विटामिन, मिनेरल और इम्यूनोग्लोबुलिन नामक पदार्थ पाये जाते हैं, जो शिशु की रोग प्रतिरोधी क्षमता को बेहतर बनाते है।
  • शिशुओं को पेट की समस्याओं से बचाता है कोलस्ट्रम। 
  • भारी मात्रा में प्रोटीन, विटामिन, मिनेरल होने के कारण कोलस्ट्रम शिशु की हडडियों के विकास के लिए और संपूर्ण विकास के लिए आवश्यक है।
  • कोलस्ट्रम शिशु को संक्रामक रोगों से लड़ने की क्षमता प्रदान करता है।
किस उम्र के बच्चे को स्तनपान कराना चाहिए
  • नवजात शिशु को पहले 6 महीनों तक सिर्फ मां का दूध देना चाहिए, यहां तक कि पानी भी नहीं देना चाहिए। क्योंकि मां के दूध में पर्याप्त मात्रा में पानी भी होता है इसलिए बच्चों को पानी की भी आवश्यकता नहीं होती। 
  • 6 महीनों कें बाद शिशुओं को मां के दूध के साथ ठोस आहार देना शुरू करना चाहिए। शिशु का आहार तैयार करते समय साफ सफार्इ का खास ख्याल रखना चाहिएा शिशु का आहार तैयार करने से पहले हाथ साबुन से ज़रूर धुलेां बच्चे को ताज़ा और पोषक आहार दे। जब शिशु मां के दूध से ठोस आहार की ओर जाता है तो ऐसे में शिशु में पोषण की कमी होने का खतरा अधिक रहता है। ठोस आहार के साथ मां का दूध कम से कम दो  साल तक शिशु को देना चाहिए।
शिशुओं को स्तनपान कराने के लाभ
  • स्तनपान करने वाले शिशुओं में पेट की समस्याएं कम होती हैं क्योंकि मां का दूध आसानी से पच जाता है।
  • मां का दूध नवजात शिशु को एलर्जी, डायरिया, उलिटयां लगने और कान के संक्रमण से बचाता है।
  • बढ़ते शिशु को उसकी आवश्यकतानुसार मां का दूध सही मात्रा में प्राप्त होता है।
  • स्तनपान बच्चों में जन्म के समय होने वाली बीमारियों के खतरे को कम करता है।
  • यह भविष्य में होने वाली बीमारियों की भी सम्भावना कम करता है जैसे डायबिटीज, पेट की समस्याएं, कैंसर, रक्तचाप आदि । 
स्तनपान से मां को होने वाले लाभ
स्तनपान के फायदे सिर्फ शिशुओं में ही नहीं, बलिक मांओं में भी देखने को मिलते है।
  1. बीमारियों का खतरा कमस्तनपान कराने वाली महिलाओं में मधुमेह, स्तन कैंसर, ओवरी के कैंसर और पोस्टपार्टम डिप्रेशन का खतरा कम रहता है।
  2. पैसों की बचतदूध गर्म करने और बाजार से खरीदने का खर्च नहीं रहता। स्तनपान करने वाले बच्चे सामान्य बच्चों की तुलना में कम बीमार पड़ते हैं , इसलिए आपको बार- बार डाक्टर के पास नहीं जाना पड़ता।
  3. तनाव कम होता है : बच्चा जहां मां की गोद में अधिक सुरक्षित अनुभव करता है, वहीं मां को भी बच्चे के साथ थोड़ा समय मिल जाता है। शिशु की त्वचा के संपर्क में आने से महिलाओं में आकिसटोसिन नामक हार्मोन बनता है, जिससे तनाव कम होता है ।
  4. अधिक वज़न से छुटकारास्तनपान कराने वाली महिलाओं को अधिक कैलोरीज की आवश्यकता होती है, जिससे वज़न कम होता है और यूटेरस भी जल्दी आकार में आ जाता है।
फार्मूला फीड और मां के दूध में अंतर
  • मां का दूध शिशुओं के लिए सर्वोत्तम आहार है क्योंकि यह एलर्जी, अस्थमा और बहुत सी बीमारियों से शिशुओं को बचाता है। फार्मूला फीड से बच्चों का पेट भरा रहता है, लेकिन उन्हें मां के दूध जितना पोषण नहीं मिलता।
  • मां का दूध हर समय सही तापमान पर उपलब्ध होता है, लेकिन फार्मूला फीड को गर्म करके तैयार करना पड़ता है। इसके अलावा अगर फार्मूला फीड सफार्इ से ना तैयार किया जाये तो इससे संक्रमण का खतरा रहता है।
  • फार्मूला फीड को एक बार बनाने के बाद दोबारा प्रयोग नहीं करना चाहिएा इसे तैयार करने में कम से  कम 5 मिनट का समय लगता है।

स्तनपान- स्पर्श की तपिश और अनुरक्ति का अहसास



शिशु के जन्म होते ही पहले स्पर्श का भी समय हो जाता है। स्पर्श की तपिश और अनुरक्ति का अहसास इतने बचपन में भी होता है। माँ की गर्मी के पास और साथ रह कर शिशु एक जुड़ाव महसूस करता है, स्वयं को सुरक्षित महसूस करता है। शिशु को साफ और मुलायम कपड़ो में सही तरीके से लपेट कर जितनी जल्दी हो सके माँ को सौंप देना चाहिये। प्रसव के आधे घंटे के अंदर स्तनपान करवाना चाहिये। यह अवधि सीज़ेरियन प्रसव से हुए बच्चों के लिये एक घंटा मानी जा सकती है।

गाय का दूध बछड़े के लिये...आपके बच्चे को आपके दूध की जरूरत है।


हर माँ का दूध उसके शिशु के लिये एकदम उपयुक्त होता है। मतलब कि अगर शिशु 28 हफ्ते में ही जन्म ले तो उसके पाचन शक्ति और जरूरत के हिसाब का सही मात्रा में प्रोटीन्स, विटामिन्स और चर्बी दूध में होती है। जैसे उस शिशु के लिये उसके जरूरत के अनुसार खास ऑर्डर पर तैयार किया गया हो।
माँ का दूध मतलब माँ के आँचल में रह कर मिलने वाली खोराक- साफ, बिना झंझट, हमेशा फ्रेश, स्टेरिलाइसेशन की आवश्यकता से दूर, हमेशा माँ के पास, माँ के साथ...सही मात्रा में प्रोटीन, विटामिन्स,ग्लूकोस, फैट। साथ में रोग प्रतिकारक शक्ति। माँ से लगातार गहराता जुड़ाव। काउ मिल्क अलर्जी से दूर।
फायदें इतने हैं कि यह समझना कठिन है कि यहाँ भी फैशन ने बाजी मार ली है। फैशन, ट्रैन्ड, फिगर और कठिनाई जैसे बहानों का इस्तेमाल कर शिशु को इससे वँचित रखना दुखद है।
नवजात शिशु को देखना हमेशा एक सुंदर अनुभव रहा है। माँ के पास गर्म , मुलायम कपड़ों में लिपटा शिशु जल्द ही बगल में मुड़कर माँ को तलाशना शुरु करता है। होठों पर जीभ फेरकर दूध की माँग रखता है। यही सही समय है स्तनपान शुरु करने का।
शिशु के लिये स्तनपान सीखी हुई एक बात है। किन्तु पहली बार बनी माँ के लिये यह एकदम नया अनुभव है। माँ को सहजता से इसकी शुरुआत करनी चाहिये। माँ की विकलता शिशु तुरंत भाँप लेता है...इसीलिये परेशान माँ का शिशु भी अक्सर चिढ़चिढ़ा हो जाता है।
पहली बार स्तन से लगाना अपने आप में एक प्रक्रिया है। इसे लैच्चिंग ऑन कहते हैं। अगर सही तरीके से स्तनपान की शुरुआत की जाये तो माँ और शिशु के बीच के रिश्ते को अच्छी और तनाव रहित शुरुआत मिल सकती है।
1.शिशु का मुँह पूरी तरह खुलवायें। हल्के से शिशु के होंठ स्तन पर लगाने से वह पूरा मुँह खोल देगा...ऐसे जैसे उबासी में खोलते हैं।
2. फिर जिस हाथ में शिशु को सँभाला है उसे पास लाकर शिशु को स्तन के पास लाना है। ध्यान रहना चाहिये कि शिशु को माँ की तरफ लाया जाये ना कि माँ को शिशु की तरफ।
ध्यान रहे कि शिशु के मसूड़े स्तन के आसपास के एरियोला (स्तन के पास गहरे रंग की त्वचा)को पूरी तरह से मुँह के अंदर लें। शिशु के होंठ बाहर की तरफ मुड़े हुए हों।
3. इतना करने पर शिशु स्तन चूस कर दूध पीने की कोशिश करेगा। 
अगर शिशु स्तन से सही तरीके से जुड़ा हो और सही तरह से चूस रहा हो तो इस प्रक्रिया में स्तन में दर्द नहीं उठना चाहिये। अगर दर्द उठे तो स्तनपान रोक कर फिर कोशिश करनी चाहिये। शिशु के स्तन पकड़ने पर वैक्यूम बनता है और सक्शन एफैक्ट से दूध खिंचता है। जब भी शिशु को स्तन से हटाना हो तो स्तन और मसूड़े के बीच उँगली ड़ाल कर पहले वैक्यूम खत्म करें। कभी भी गलत तरीके से स्तनपान जारी ना रखे। इससे स्तन पर चीरे पड़ने का डर रहता है और शिशु को भी पर्याप्त मात्रा में दूध नहीं मिलता। 
माँ और शिशु के रिश्ते का एक अहम पहलू है स्तनपान। माँ की खुशबू, सपर्श की तपिश, और माँ की गोद की सुरक्षा...इन सबके लिये पहला अनुभव है स्तनपान। स्वाभाविक क्रिया है और जितना सहजता और आनंद से निभाया जाये उतनी ही संतुष्टि दोनो को मिलती है।


छह महीने तक शिशु को मात्र स्तनपान देने से उसकी सभी खोराक की जरूरत पूरी हो जाती है। सिर्फ माँ का दूध उसके लिये पर्याप्त है। इसे एक्सक्लूसिव ब्रेस्टफीड़िंग कहते हैं। छह महीने से माँ के दूध के अलावा कुछ और भी शुरू किया जाना चाहिये। स्तनपान मात्र खोराक ही नहीं पूरा करता किंतु इससे शिशु का पहला स्नेह संबंध बनता है। 
कहते हैं भगवान ने माँ को इसलिये बनाया क्योंकि हर जगह हर समय हर शिशु के पास नहीं रह सकता....काश हर शिशु के पास उसका अपना भगवान ......उसकी माँ हो....जो उसे ममता से सँभाल,दूध से सींच,स्नेह से छू....उसको उसकी अपनी पहचान दे.....

Breastfeeding Benefits : from Top to Bottom


  • Brain. Higher IQ in breastfed children. Cholesterol and other types of fat in human milk support the growth of nerve tissue.

  • Eyes. Visual acuity is higher in babies fed human milk.

  • Ears. Breastfed babies get fewer ear infections.

  • Mouth. Less need for orthodontics in children breastfed more than a year. Improved muscle development of face from suckling at the breast. Subtle changes in the taste of human milk prepare babies to accept a variety of solid foods.

  • Throat. Children who are breastfed are less likely to require tonsillectomies.

  • Respiratory system. Evidence shows that breastfed babies have fewer and less severe upper respiratory infections, less wheezing, less pneumonia and less influenza.

  • Heart and circulatory system. Evidence suggests that breastfed children may have lower cholesterol as adults. Heart rates are lower in breastfed infants.

  • Digestive system. Less diarrhea, fewer gastrointestinal infections in babies who are breastfeeding. Six months or more of exclusive breastfeeding reduces risk of food allergies. Also, less risk of Crohn's disease and ulcerative colitis in adulthood.

  • Immune system. Breastfed babies respond better to vaccinations. Human milk helps to mature baby's own immune system. Breastfeeding decreases the risk of childhood cancer.

  • Endocrine system. Reduced risk of getting diabetes.

  • Kidneys. With less salt and less protein, human milk is easier on a baby's kidneys.

  • Appendix. Children with acute appendicitis are less likely to have been breastfed.

  • Urinary tract. Fewer infections in breastfed infants.

  • Joints and muscles. Juvenile rheumatoid arthritis is less common in children who were breastfed.

  • Skin. Less allergic eczema in breastfed infants.

  • Growth. Breastfed babies are leaner at one year of age and less likely to be obese later in life.

  • Bowels. Less constipation. Stools of breastfed babies have a less-offensive odor.

Breastfeeding and Bottlefeeding : Nipple Confusion

There are some basic mechanical differences between how a baby gets milk from a bottle and how a baby gets milk out of the breast. Giving bottlesor pacifiersto young, breastfeeding babies often leads to nipple confusion. Baby tries to use the bottle-feeding technique on the breast and has difficulty latching-on and sucking. Baby gets very frustrated, and so does mother. Nipple confusion can even lead to baby refusing the breast. Here's an explanation.
To get milk from the breast, baby must coordinate tongue and jaw movements in a sucking motion that's unique to breastfeeding.
  • When baby latches onto the breast, he opens his mouth wide and draws the very stretchable nipple and areolar tissue far back into his mouth.
  • The tongue holds the breast tissue against the roof of baby's mouth while forming a trough beneath the nipple and areola.
  • The gums compress the milk sinuses underneath the areola (the pigmented area around the nipple) while the tongue rhythmically "milks" the breast with a wave-like motion from front to back, drawing the milk from the areola and the nipple.
  • Since the nipple is far back in baby's mouth, it's not compressed by the gums, so it's less likely to get sore.
Babies suck from a bottle entirely differently. Thanks to gravity, milk flows from a bottle so easily that baby does not have to suck "correctly" to get milk.

  • He doesn't have to open his mouth as wide or correctly turn out the lips to form a tight seal.
  • The bottle nipple does not need to be far back into the mouth, nor is the milking action of the tongue necessary.
  • Baby can lazily gum the nubbin of the rubber and suck with only his lips.
  • When the milk comes out too fast, baby may thrust his tongue forward and upward, to stop the flow from the nipple.
  • Milk keeps on coming during feedings from bottles--whether or not baby sucks--so there are no pauses to rest during bottle-feedings.
Problems occur when babies apply the lessons learned from bottle-feeding to nursing at the breast. When you compare the illustration of sucking at an artificial nipple with the illustration of sucking at the breast, you will see that if baby sucks from the breast the same way he does the bottle, the tongue and the gums will traumatize mother's nipple.

  • Babies who get bottles soon after birth may thrust their tongue upward during sucking and push the breast nipple out of their mouth.
  • They don't open their mouths wide enough when latching-on, so they suck only the tip of the nipple. They don't get enough milk, and mother's nipples get sore.
  • Baby becomes accustomed to the immediate flow of milk that comes from the bottle; at the breast, babies have to suck for a minute or two to stimulate mother's milk ejection reflex and get the milk flowing.
Does this mean that bottle-feeding is easier than breastfeeding? Yes, and no. Bottles require less sucking finesse and less effort. However, studies comparing premature infants during bottle-feedings and during breastfeedings have shown that breastfeeding is actually less stressful.


  • Babies' breathing and heart rate are more stable during feedings at the breast.
  • Babies have more control over the milk flow and can establish a more regular rhythm of sucking, swallowing, and pausing.
  • Feeding at the breast also requires less energy.
PREVENTING NIPPLE CONFUSION


It is easier to prevent nipple confusion than to fix it--though it is a problem that can be solved, should it occur (see below). Breastfed babies should not be given artificial nipples during the first three to four weeks when they are learning and perfecting their breastfeeding skills. Avoiding artificial nipples means avoiding pacifiers as well as bottles. Supplements, if medically necessary, can be given in ways that don't involve artificial nipples. 
Will it be more difficult to introduce the bottle later? Many mothers, because they are going back to work or because they eventually plan to get out for a few hours by themselves, want their breastfed babies to accept feedings from bottles. They have heard stories of babies who adamantly refused anything but the breast. Getting baby to accept a bottle at age two or three months may take some patience, but most babies will catch on after a few tries. (Babies can also be fed with alternatives to bottles when mother is gone.) While introducing the bottle at one or two weeks of age may insure that baby accepts the bottle later, you're taking a risk. Some babies easily go back and forth between breast and bottle, but many others do not. Don't jeopardize your breastfeeding relationship when it has barely begun.

UN-CONFUSING THE NIPPLE-CONFUSED BABY


  • Banish bottles and pacifiers. Even if your baby will eventually have to learn to use the bottle because you are returning to work, don't ask him to learn both skills at the same time.
  • If supplements are needed, they can be given in ways that don't use artificial nipples.

  • Reacquaint baby with the pleasures of breastfeeding. Give her lots of skin-to-skin contact. Carry her in a sling near the breast between feedings.
  • Breastfeed when baby is calm, usually in the morning or upon awakening from a nap. Don't wait until baby is ravenously hungry--she'll be in no mood to try something new.
  • Review the latch-on basics. Be sure that baby is positioned properly in your arms. Wait until her mouth is wide open and her tongue is down before latching her on to the breast.
  • Show and tell. Open your mouth as you say "open" to baby during latch-on. Even newborns can imitate adult facial expressions.
  • Provide baby with instant gratification at the breast. Use a breast pump or manual expression to stimulate your milk ejection reflex and get the milk flowing before latching baby on. She'll be rewarded with a hearty flow of milk after the first few sucks.
  • Use an eyedropper or feeding syringe to drip milk into baby's mouth as she latches on to the breast. (Get some help with this one.) This may encourage baby to stay latched-on and to continue sucking.

Babies often act puzzled or uncertain when they are re-introduced to the breast. Be patient. Praise your baby for every tiny step she takes back to breastfeeding. It may take a few days to woo baby back to the breast, but you can do it.

1.  कितने बार दूध पिलाना चाहिए?

  • पहले दो महीनों में आपका बच्चा 8 से 12 बार प्रतीदिन स्तनपान कर सकता है| यह 2 से 3 घंटे के अंतराल पर हो सकता है| कभी-कभार 4 घंटे तक भी बच्चा सो सकता है| लेकिन उससे अधिक होने पर, बच्चे को नींद से जगा कर दूध पिलाने का कोशिश करना चाहिए| नवजात शिशु, रात में भी 1 से 2 बार तक दूध पी सकता है |
  • अगर आपका बच्चा 24 घंटे में 8 बार से कम दूध पी रहा है तो उसका वजन कम हो सकता है|
  • अगर आपका बच्चा 6 घंटे से ज्यादा लगातार सो रहा है, तो उसका वजन कम हो सकता है|
  • दूध पीते समय दूध को निगलना
  • दूध पीने के बाद संतुष्ट प्रतीत होना
  • बच्चा तय करता है कि उसे कितना देर तक दूध पीना है। यह 10 से 20 मिनट तक हो सकता है। यह एक तरफ या दोनों तरफ हो सकता है।

2.  बच्चे का वजन
सभी नवजात शिशु जन्म के तुरंत बाद वजन खोते हैं, और लगभग २ हफ्ता तक के उम्र में, उनका वजन जन्म के वजन के बराबर या उससे अधिक होना चाहिए| जो बच्चा समय से पहले होते हैं, या प्रिमेच्योर (premature) होते हैं, उनको वजन बराबर करने में ३ हफ्ता लग सकता है| अगर किसी बच्चा का वजन २ हफ्ते बाद, जन्म के वजन से कम है, तो डाक्टर से जरूर दिखलाना चाहिए कि क्या आपके बच्चे को पार्याप्त पोषण मिल रहा है कि नहीं | २ हफ्ते पर नवजात शिशु का वजन जांच जरूर करायें |
3.  पैशाब और पैखाना

  • स्वस्थ बच्चा प्रतीदिन 8 से अधिक बार पैशाब करता है|
  • पहले महीना में स्वस्थ नवजात शिशु दिन में कम से कम एक बार पैखाना करता है| दूसरे महीने में यह कम हो सकता है| हर बच्चे में यह अलग अलग हो सकता है कि किसी बच्चे को दिन में 5 बार भी पैखाना हो रहा है, तो किसी दूसरे को 7 दिन में 1 बार पैखाना हो रहा है| महत्वपूर्ण बात यह होता है कि क्या बच्चा स्वस्थ दिख रहा है कि नहीं, अर्थात ठीक तरह से दूध पी रहा है, पैशाब हो रहा है, कोई उल्टी नहीं हो रहा है, पेट नहीं फूला हुआ है और अन्य कोई समस्या नहीं है| यह भी देखना जरूरी है कि अधिक बार पैखाना कहीं दस्त या डाईर्हेया (diarrhea) तो नहीं है और कम पैखाना कहीं कब्जीयत या कोंसटीपेशन (constipation) तो नहीं है|

4.  नवजात शिशु बहुत रोता है
साथ ही कभी कभार स्वस्थ बच्चे में भी हर रोज अत्याधिक रोना हो सकता है, जिसे कोलिक (colic) कहते हैं|

स्तनपान के अनेक फ़ायदे हैं| तमाम रिसर्च यही कहते हैं कि नवजात शिशु के लिए मां के दूध से बेहतर और कोई भी दूध नहीं होता है| इससे दोनों मां और बच्चे को अनेक लाभ पहुंचता है| इसके टक्कर में न तो कोई जानवर (गाय, भैंस इत्यादि) का दूध है और न ही कोई कृत्रिम (artificial) दूध है|
2.  क्या कृत्रिम दूध (artificial milk), मां के दूध (maternal milk) के जैसा नहीं होता है?
कोई भी कृत्रिम दूध, मां के दूध के गुणवत्ता (properties), का अनुकरण (copy) करने का कोशिश कर सकता है, लेकिन सही माने में यह अनुकरण हो ही नहीं सकता है| यह इस लिए कि मां के दूध में अनेक गुणधर्म हैं, जिनका अनुकरण करना नामुमकिन है | कृत्रिम दूध में मां के दूध के जैसा सामग्री "कार्बोहाइड्रेट (carbohydrate), प्रोटीन (Protein), फ़ेट (Fat) और विटामिन इत्यादि" डाल दिए जाते हैं, किंतु इनका मात्रा नियत (fixed) रहता है| दूसरे तरफ मां के दूध में इनका मात्रा बदलते रहता है| कभी मां का दूध गाढा रहता है तो कभी पतला, कभी दूध कम होता है तो कभी अधिक, जन्म के तुरंत बाद और जन्म के कुछ हफ्तों बाद या महीनों बाद बदला रहता है| इससे दूध में उपस्थित सामग्री का मात्रा बदलते रहता है, और यह प्रकृति का बनाया गया नियम है कि मां का दूध बच्चे के उम्र के साथ बदलते (adjust) होते रहता है|

स्तनपान के विभिन्न तरीके/Breeastfeeding Positions


1.  ब्रेस्टफीडिंग के विभिन्न तरीके

ब्रेस्ट्फीडिंग के विभिन्न तरीके होते हैं। आप इसमें से जो भी आपको आसान लगे, वह अपना सकते हैं। यहां उन तरीकों के बारे में बताया जा रहा है। जो भी तरीका अपनायें, यह जानना जरूरी है कि बच्चा सही तरह से दूध पी रहा है, अन्यथा नवजात शिशु का अच्छे तरह से पोष्ण नहीं होगा। साथ ही, गलत तरह से दूध पिलाने से, स्तन में अनेक तरह का परेशानी हो सकता है, जो आगे जाकर बच्चे को दूध पिलाने में दिक्कत कर सकता है। ये स्थिती हो सकता है, जैसे कि निप्पल में दर्द, कटना, खून बहना, अत्याधिक दूध भरा रहना इत्यादि। इन सभी के बारे में अन्य जगह बताया गया है।
2.  “लेचिंग” (latching) का क्या मतलब होता है?
“सही तरह से बच्चा को दूध पिलाने” को लेचिंग कहते हैं। सही तरह के लेचिंग के समय, बच्चे का मुंह स्तन पर इस तरह लगा होता है कि बच्चे का मुंह बड़ा खुला होता है, और उसका नीचे का मसूड़ा, ब्रेस्ट के निप्पल और एरियोला का घेरा होता है। इसके बारे में यहां बताया गया है।

इस स्थिती में आप अपने बच्चे के सिर को, अपने कोहनी के अन्दर से, सहारा दे सकते हैं। दूसरे हाथ के सहायता से अपना स्तन बच्चे के मुंह से लगा सकते हैं। यह सबसे ज्यादा प्रचलित तरीका। शुरू के हफ्तों में यह तरीका अपनाने में मुश्किल हो सकता है, क्योंकि बच्चा का गर्दन कमजोर होता है, और इस स्थिती में कोई अन्य तरीका आप अपना सकते हैं। बाद में, जब बच्चा का गर्दन मजबूत हो जाता है, तो अपना सिर उठा कर आपके स्तन से लगा सकता है, और तब यह तरीका बहुत ही सुविधाजनक लग सकता है।

4.  क्रोस-ओवर होल्ड (Cross-over hold) क्या होता है?

Breastfeeding Myths-I

1. Many women do not produce enough milk.
Not true! The vast majority of women produce more than enough milk. Indeed, anoverabundance of milk is common. Most babies that gain too slowly, or lose weight, do sonot because the mother does not have enough milk, but because the baby does not get the milk that the mother has. The usual reason that the baby does not get the milk that is available is that he is poorly latched onto the breast. This is why it is so important that the mother be shown, on the first day, how to latch a baby on properly, by someone who knows what they are doing.




2. It is normal for breastfeeding to hurt.

Not true! Though some tenderness during the first few days is relatively common, this should be a temporary situation which lasts only a few days and should never be so bad that the mother dreads nursing. Any pain that is more than mild is abnormal and is almost always due to the baby latching on poorly. Any nipple pain that is not getting better by day 3 or 4 or lasts beyond 5 or 6 days should not be ignored. A new onset of pain when things have been going well for a while may be due to a yeast infection of the nipples. Limiting feeding time does not prevent soreness.  




3. There is no (not enough) milk during the first 3 or 4 days after birth.

Not true! It often seems like that because the baby is not latched on properly and therefore is unable to get the milk. Once the mother's milk is abundant, a baby can latch on poorly and still may get plenty of milk. However, during the first few days, the baby who is latched on poorly cannot get milk. This accounts for "but he's been on the breast for 2 hours and is still hungry when I take him off". By not latching on well, the baby is unable to get the mother's first milk, called colostrum. Anyone who suggests you pump your milk to know how much colostrum there is, does not understand breastfeeding, and should be politely ignored. 




4. A baby should be on the breast 20 (10, 15, 7.6) minutes on each side.

Not true! However, a distinction needs to be made between "being on the breast" and "breastfeeding". If a baby is actually drinking for most of 15-20 minutes on the first side, he may not want to take the second side at all. If he drinks only a minute on the first side, and then nibbles or sleeps, and does the same on the other, no amount of time will be enough. The baby will breastfeed better and longer if he is latched on properly. He can also be helped to breastfeed longer if the mother compresses the breast to keep the flow of milk going, once he no longer swallows on his own (Handout #15 Breast Compression). Thus it is obvious that the rule of thumb that "the baby gets 90% of the milk in the breast in the first 10 minutes" is equally hopelessly wrong. 




5. A breastfeeding baby needs extra water in hot weather.

Not true! Breastmilk contains all the water a baby needs. 




6. Breastfeeding babies need extra vitamin D.

Not true! Except in extraordinary circumstances (for example, if the mother herself was vitamin D deficient during the pregnancy). The baby stores vitamin D during the pregnancy, and a little outside exposure, on a regular basis, gives the baby all the vitamin D he needs.




7. A mother should wash her nipples each time before feeding the baby.

Not true! Formula feeding requires careful attention to cleanliness because formula not only does not protect the baby against infection, but also is actually a good breeding ground for bacteria and can also be easily contaminated. On the other hand, breastmilk protects the baby against infection. Washing nipples before each feeding makes breastfeeding unnecessarily complicated and washes away protective oils from the nipple.




8. Pumping is a good way of knowing how much milk the mother has.

Not true! How much milk can be pumped depends on many factors, including the mother's stress level. The baby who nurses well can get much more milk than his mother can pump. Pumping only tells you have much you can pump. 




9. Breastmilk does not contain enough iron for the baby's needs.

Not true! Breastmilk contains just enough iron for the baby's needs. If the baby is full term he will get enough iron from breastmilk to last him at least the first 6 months. Formulas contain too much iron, but this quantity may be necessary to ensure the baby absorbs enough to prevent iron deficiency. The iron in formula is poorly absorbed, and most of it, the baby poops out. Generally, there is no need to add other foods to breastmilk before about 6 months of age. 




10. It is easier to bottle feed than to breastfeed.

Not true! Or, this should not be true. However, breastfeeding is made difficult because women often do not receive the help they should to get started properly. A poor start can indeed make breastfeeding difficult. But a poor start can also be overcome. Breastfeeding is often more difficult at first, due to a poor start, but usually becomes easier later. 




11. Breastfeeding ties the mother down.

Not true! But it depends how you look at it. A baby can be nursed anywhere, anytime, and thus breastfeeding is liberating for the mother. No need to drag around bottles or formula. No need to worry about where to warm up the milk. No need to worry about sterility. No need to worry about how your baby is, because he is with you.




12. There is no way to know how much breastmilk the baby is getting.

Not true! There is no easy way to measure how much the baby is getting, but this does not mean that you cannot know if the baby is getting enough. The best way to know is that the baby actually drinks at the breast for several minutes at each feeding (open—pause—close type of suck). Other ways also help show that the baby is getting plenty.




13. Modern formulas are almost the same as breastmilk.

Not true! The same claim was made in 1900 and before. Modern formulas are only superficially similar to breastmilk. Every correction of a deficiency in formulas is advertised as an advance. Fundamentally they are inexact copies based on outdated andincomplete knowledge of what breastmilk is. Formulas contain no antibodies, no living cells, no enzymes, no hormones. They contain much more aluminum, manganese, cadmium and iron than breastmilk. They contain significantly more protein than breastmilk. The proteins and fats are fundamentally different from those in breastmilk. Formulas do not vary from the beginning of the feed to the end of the feed, or from day 1 to day 7 to day 30, or from woman to woman, or from baby to baby... Your breastmilk is made as required to suit your baby. Formulas are made to suit every baby, and thus nobaby. Formulas succeed only at making babies grow well, usually, but there is more to breastfeeding than getting the baby to grow quickly. 




14. If the mother has an infection she should stop breastfeeding.

Not true! With very, very few exceptions, the baby will be protected by the mother's continuing to breastfeed. By the time the mother has fever (or cough, vomiting, diarrhea, rash, etc) she has already given the baby the infection, since she has been infectious for several days before she even knew she was sick. The baby's best protection against getting the infection is for the mother to continue breastfeeding. If the baby does get sick, he will be less sick if the mother continues breastfeeding. Besides, maybe it was the baby who gave the infection to the mother, but the baby did not show signs of illness because he was breastfeeding. Also, breast infections, including breast abscess, though painful, are not reasons to stop breastfeeding. Indeed, the infection is likely to settle more quickly if the mother continues breastfeeding on the affected side.




15. If the baby has diarrhea or vomiting, the mother should stop breastfeeding.

Not true! The best medicine for a baby's gut infection is breastfeeding. Stop other foods for a short time, but continue breastfeeding. Breastmilk is the only fluid your baby requires when he has diarrhea and/or vomiting, except under exceptional circumstances. The push to use "oral rehydrating solutions" is mainly a push by the formula (and oral rehydrating solutions)manufacturers to make even more money. The baby is comforted by the breastfeeding, and the mother is comforted by the baby's breastfeeding..




16. If the mother is taking medicine she should not breastfeed.

Not true! There are very very few medicines that a mother cannot take safely while breastfeeding. A very small amount of most medicines appears in the milk, but usually in such small quantities that there is no concern. If a medicine is truly of concern, there are usually equally effective, alternative medicines which are safe. The loss of benefit of breastfeeding for both the mother and the baby must be taken into account when weighing if breastfeeding should be continued.
